Sinus grafting is recognized as an effective treatment modality with high implant survival rates having been reported. The lateral approach technique entails opening a lateral window in the bone and elevating the sinus membrane from the inferior aspect of the maxillary sinus so graft material can be placed between the membrane and the inferior wall of the sinus. 

Once the lateral opening is made and access to the sinus is obtained, the sinus membrane is carefully separated from its bony attachment using blunt dissection. The membrane is delicately elevated to permit the placement of graft material between the membrane itself and the bony sinus floor. Care also should be taken not to interfere with sinus drainage and not to tear or perforate the membrane. This external (lateral) approach continues to be a popularly used surgical technique.

Irrespective of the surgical approach or materials used for grafting the maxillary sinus, it is important for the surgeon to know the anatomy of the surgical field, be able to identify and evaluate the maxillary sinus membrane, understand the surgical management procedures, and be familiar with evidence-based data related to long-term results when performing a sinus graft procedure.
