Privacy Policy

This privacy policy (the “Privacy Policy”) covers the collection, use and disclosure of the personal data of any Associate Fellow (as defined in the Terms of Use) using FOR’s web-based platform (the “Platform”) or FOR’s mobile application (the “FOR Application”) as well as the cookies and other trackers that FOR uses in the way described below.

 

Your privacy is very important to FOR. FOR, and any service provider that may operate the Platform or the FOR Application on behalf of FOR, will use all reasonable measures to keep your personal data secure. By using the Platform or the FOR Application and submitting personal data through the Platform or the FOR Application, you agree to the collection, processing and storing of your data in accordance with this Privacy Policy. 


This Privacy Policy forms an integral part of the Terms of Use of the Platform and the FOR Application. 

 

a) Cookies

Like many websites, FOR uses cookies to collect visitor information, for example to enable us to identify your device or to remember selected features from a previous session (e.g. through Google Analytics, see below). Cookies are alphanumeric identifiers that FOR’s Platform transfers to your computer's hard drive through your web browser. It is possible to prevent cookies from being used in your browser by turning the feature off in your browser settings.


We use the following cookies:

  • Strictly necessary cookies. These are cookies or other tracking mechanisms necessary to make our Platform function as intended.
  • Analytical/performance cookies. They allow us to recognize and count the number of visitors and to see how visitors move around our Platform when they are using it. This helps us to improve the way our Platform works, for example, by ensuring that users are easily finding what they are looking for.
  • Functionality cookies. These cookies enable us to personalize our content for you, greet you by name and remember your preferences (for example, your choice of language or region). They may also be used to recognize you when you return to our Platform.
  • Third-party advertising cookies. These cookies placed on your device by third parties record your visit to our Platform, the pages you have visited and the links you have followed. They use this information to make the advertising displayed on other sites more relevant to your interests.

Except for strictly necessary cookies and functionality cookies that we keep only for the duration of your session, we will ask you for your consent to our use of cookies when you first visit our Platform.  We may assume that you consented from ticking a box and clicking on an “I agree” (or similar button) or by continuing to navigate on our Platform despite a clear and visible notice on our Platform that we use cookies. If you agree, we will place an opt-in cookie on your device.  If you disagree, we will not use cookies, however, you may not profit from all of the functionalities of the website. If you change your mind, simply delete the cookies on your device (please refer to the provider’s documentation) and you will be asked again if you agree or disagree with our use of cookies when you visit our Platform next.


Some of our partners and software vendors use cookies on our Platform (e.g. for website analytics purposes). While we strive to protect the personal data of our Platform users and thoroughly review the security and privacy practices of our Platform partners, we have no access to or control over these cookies. As such, this privacy statement covers the use of cookies by us only and does not cover the use of cookies by any business partners or software vendors.


The Third Party cookies that we may use on the Platform are: 

  • Google Analytics helps to compile statistics about the use of our Platform. This includes data such as how many people have visited our website, what type of technology they are using (e.g. Mac or Windows which helps to identify when our site isn't working as it should for particular technologies), what demographics and interests the users have, how long they spend on the site, what page they look at etc. This helps us to continuously improve our Platform.   • Facebook Pixel: FOR enables Facebook to track our visitors’ behavior on the FOR website by the use of pixels and other snippets. You may opt out of this pixel tracking by clicking on the following link: http://www.aboutads.info/choices.
  • Intercom: FOR utilizes Intercom to collect data for analytics purposes when you visit our Platform. Intercom analyzes your use of our Platform and/or product and tracks our relationship so that we can improve FOR's service to you. We may also use Intercom as a medium for communication, either through email, or through messages. Intercom collects publicly available contact and social information related to you, such as your email address, gender, company, job title, photos, website URLs, social network handles and physical addresses, to enhance your user experience. b) Log files

Like most standard websites, we use log files to automatically log information such as IP addresses, browser type, internet service provider (“ISP”), referring/exit pages, platform type, date/time stamp, and number of clicks. We log this information to manage traffic loads, identify the content most commonly accessed and determine information technology requirements for providing reliable service, or other web management, deployment, tracking, maintenance and similar activities. We do not use this information to identify you.

 

c) What information do we collect?

When you register as an Associate Fellow onto the Platform you provide FOR with personal data, which can include your first and last name, date and place of birth, address, email address, phone number, languages, profession, education, and more generally any information that identifies you (your “Personal Data”). Some information is necessary for us to create your account (name, email address) and some information allows us to know you and serve you better, you can decide whether you want to add it to your Associate Fellow profile or not. You have the right to access and rectify at any time your Personal Data collected by FOR and to request its deletion, should you decide not to be an Associate Fellow anymore, please contact FOR at the address indicated below in section f.

 

d) What do we do with your data?

FOR uses your Personal Data in order to improve and administer the Platform, to create your account, to communicate with you, and more generally to provide you with services available through the Platform. FOR will store and process your Personal Data in accordance with this Privacy Policy. FOR must collect and use your Personal Data in order to register you as an Associate Fellow and grant you full access to the features of the Platform. FOR may also use your Personal Data to contact you to announce FOR-related events, or any other FOR-related services We may also anonymize your Data in order to later make statistics on the use of our Platform. Such anonymization is irreversible, ensuring that the statistical data cannot be linked to any particular user. 


FOR may ask you questions in order to serve you better, e.g. about your practice or what you would like to find as content on the Platform. These questions are optional for you to answer, however, if you do answer, we will record them in order to tailor our content to your wishes. 


We do not spam, never sell or rent email addresses, and there is an easy unsubscribe in every email you will receive. Alternately you may remove your address from our mailing lists by sending us an email to the address indicated below.

 

e) How do we secure your data and when do we disclose it?

Processors
FOR implements all appropriate technical and organizational measures to protect your Personal Data against accidental or unlawful destruction or accidental loss, alteration, unauthorized disclosure or access for third parties and against other unlawful forms of processing. Additionally, FOR may request a third party service provider to collect, store and process your Personal Data on FOR’s behalf (for example to maintain the IT system).  These measures will also be implemented by such processor of your Personal Data authorized by FOR in accordance with this Privacy Policy. FOR may transfer the data it collects to processors within the EEA, or outside the EEA to processors with which FOR has secured an agreement ensuring an adequate level of protection of your data (for example, the European Commission Standard Contractual Clauses (http://ec.europa.eu/justice/data-protection/international-transfers/transfer/index_en.htm)). FOR will take all appropriate measures to ensure that this third party service provider will use the same level of care and security that FOR uses in relation to your Personal Data.


Mandatory disclosure
You acknowledge and agree that FOR may use and/or disclose your Personal Data if FOR deems that such use and/or disclosure is reasonably necessary to (1) comply with legal process, (2) enforce the Terms of Use of the Platform or the FOR Application or the Terms of Participation to the Platform, or (3) protect rights, property or personal safety of FOR, the Users of the Platform, or the public.


Transfer of data to third parties
Unless in these cases where the transfer of your data to processors is necessary for the administration of the FOR Platform or is mandatory, FOR will never transfer your Personal Data to third party companies unless you have explicitly accepted such transfer.


Links to third-party websites
FOR Platform may contain links to other websites. However, once you have used these links to leave our website, you should note that we do not have any control over that other website. Therefore, we cannot be responsible for the protection and privacy of any information which you provide whilst visiting such websites and such websites are not governed by this privacy statement. You should exercise caution and look at the privacy statement applicable to the website in question.

 

Patient information
No Submission (as defined in the Terms of Use) or any other content submitted by you shall contain any personal information concerning a patient, unless this information is anonymous. You are solely responsible for ensuring compliance with this provision, and in no event shall FOR nor any person related to it, be held liable for any loss or damage, whether direct or indirect, that may arise from the Submission of patient data that is not anonymized. FOR will delete all data and Submissions sent by you in violation of this provision.
