A common method of providing access to the bone for implant placement is to make an incision through the soft tissue with a scalpel. Initially, surgeons made these incisions in the mucobuccal fold but subsequent “pain and significant edema” were associated with such alveolar mucosa incisions. As a result, the incision design for most dental implant surgeries is now more frequently made in keratinized tissue at the crest of the edentulous ridge.

When a surgical flap is required, primary closure of the soft tissue can be achieved using different suturing techniques with single interrupted sutures often being used. Obtaining “tension-free” closure of the flap is more important than the suturing technique used.

The aim of this chapter is to compare minimally invasive implant surgery procedures with traditional surgical flap designs. Minimal access surgery, also referred to a “flapless” surgical technique, has made remarkable inroads in numerous areas in medicine. Laparoscopic surgery, robotics, ophthalmic surgery as well as use of stereotactic neurosurgery are just a few examples of minimally invasive medical procedures. Since their introduction, these enhancements have revolutionized the practice of surgery by decreasing morbidity, blood loss as well as the number of patient hospital stays. In some instances, these services have been found to be more cost effective than traditional approaches.
